What this document is

“คำร้องทั่วไป แบบ ก.5” is the general petition (Form K.5) of the Marine Department of Thailand (กรมเจ้าท่า, MD). In this published version the subject is pre-printed: “ขอต่ออายุใบอนุญาตใช้เรือ” — renewal of a vessel operating licence (ใบอนุญาตใช้เรือ), the licence to use a Thai-registered boat. The place of writing is also pre-printed: the Ship Registration Division (กองทะเบียนเรือ).

The form is in Thai only and has one page. Its header still shows the department’s former name, “กรมการขนส่งทางน้ำและพาณิชยนาวี” (Department of Water Transport and Merchant Marine) — this is how the Marine Department itself publishes it. The same K.5 form with other pre-printed subjects is used for other actions (for example, a change of vessel name or a mortgage); this version is for licence renewal only.

Who files it

  • the owner or operator of a Thai-registered boat — for example a charter or private boat in Phuket, Pattaya (Chonburi) or Koh Samui;
  • anyone filing on the owner’s behalf — a captain, company staff or an agent: according to the Marine Department, any person may be the petitioner for a licence renewal, and no power of attorney is needed;
  • registering a vessel under the Thai flag in one’s own name is generally limited to Thai nationals and Thai companies, so foreign residents typically deal with this form as a director or employee of a Thai company, or as the person filing for the owner.

Foreign-flagged yachts visiting Thailand do not use this form; they go through clearance with customs, immigration and the harbour master instead.

What the form asks for

  • date (day, month, Buddhist Era year, พ.ศ.);
  • the petitioner’s first name, surname, nationality and age;
  • address: house number, moo, road, tambon/khwaeng, amphoe/khet, province, phone (there is no soi line);
  • the vessel name and its registration number;
  • the date the current vessel operating licence expires;
  • signature with the petitioner’s name in brackets.

A note at the bottom of the form says the name, surname and address of the petitioner (who may be anyone) go in item 1, and the person who signs must be the petitioner named in item 1.

Documents usually brought along

  • the current (expiring) vessel operating licence;
  • the vessel registration certificate (ใบทะเบียนเรือ);
  • a valid vessel inspection certificate (ใบสำคัญรับรองการตรวจเรือ) — the Marine Department says it must accompany the relevant requests;
  • the petitioner’s ID card or passport (copy).

The Marine Department asks for documents to be arranged in order. The licence fee is set by Marine Department regulations and depends on the vessel’s size. Check the exact list, timing and amounts with the Marine Office.
