What this document is

Form ขส.บ. 10 ท.-12 “แบบคำขอแก้ไขเพิ่มเติมรายการในทะเบียนรถ” is an application to Thailand’s Department of Land Transport (DLT) to change the particulars in a vehicle registration book (เล่มทะเบียน). It is used when the owner moves or changes name, when the vehicle is repainted, gets a new engine, is converted to gas, and for other changes.

The form is in Thai and has two pages: the owner fills in the first; the second is the inspection record completed by an officer. The form notes that if the vehicle itself or any part of it is changed, it must be brought for inspection.

Who files it

  • the vehicle owner recorded in the registration book;
  • a representative with a DLT power of attorney;
  • if the vehicle is on hire purchase, the leasing company is the owner in the book — check the procedure with it.

What the form asks for

  • date (Buddhist Era year, พ.ศ.);
  • name, age, nationality, address and phone of the applicant;
  • plate number and province;
  • what changes: engine (new make and number), colour, fuel, owner’s name, owner’s address, other;
  • up to six attached documents.

What is usually attached

  • the vehicle registration book;
  • a copy of the passport (a Thai citizen — ID card);
  • after a move — a residence certificate or a copy of the house registration;
  • for a name change — the name change document;
  • for a new engine or gas kit — the documents for the engine or equipment;
  • a power of attorney if a representative files.

The exact list, fees and processing time are not on the form — check with the DLT office where the vehicle is registered.

Frequently asked questions

Do I have to update the address in the registration after moving?

Yes, the owner’s address in the registration book should be current. This form is filed with “change of owner’s address” ticked.

Can several changes be ticked at once?

Yes, the circles are independent — for example a repaint and a gas conversion in one application.

Do I need to bring the vehicle?

If the vehicle itself changes (engine, colour, fuel), the form requires an inspection. For an address or name change, documents are usually enough.

What goes on page 2?

Nothing — it is the inspection record completed by a DLT officer.

Can a representative file it?

Yes, with a DLT power of attorney, listed among the attachments.

How much does it cost and how long does it take?

The form does not state fees or processing time. Check with the DLT office where the vehicle is registered.
