What this document is
TM.28 (ตม.28 แบบการแจ้งย้ายสถานที่พักอาศัยและเดินทางไปอยู่ต่างจังหวัดเกิน 24 ชั่วโมงของบุคคลต่างด้าว) is the form a foreigner uses to tell Thai immigration that they have changed their address or are staying in another province for more than 24 hours.
The form is bilingual (Thai and English) and has one page. It is addressed to the Immigration Officer. The lower part, the receipt of notification, is for official use only.
Who files it and when
- a foreigner in Thailand who has moved to a new address;
- a foreigner who travels to another province and stays there for more than 24 hours;
- according to the note on the form, a change of address must be notified within 24 hours;
- according to the note on the form, a stay of over 24 hours in another province must be reported within 48 hours of arrival there;
- TM.28 is filed by the foreigner personally; it is different from TM.30, which the owner of the accommodation files. Ask the immigration office which notification applies to you.
What the form asks for
- place and date of writing (Buddhist Era year, พ.ศ.);
- full name in block letters and nationality;
- type of visa, date of entry into Thailand, conveyance (flight), passport number, arrival card number (if any);
- new address: house number, soi/road, tambon, amphoe, province;
- phone number and the date since which you live at the new address;
- signature by hand.
Documents usually brought along
- passport and copies of the photo page, visa and latest entry or extension stamp;
- proof of the new address if the office asks for it (for example, a lease agreement).
