What this document is

Form ขส.บ. 10 อ.-1-ท “แบบคำขออื่นๆ” is a general application to the Registrar (นายทะเบียน) of Thailand’s Department of Land Transport for requests under the Motor Vehicle Act that have no form of their own. The request itself is written as free text.

Typical cases: a duplicate of a lost or damaged registration book or licence plates, a certificate or certified copy of the registration details (ownership history), a vehicle document for travel to Laos or Cambodia, a replacement driving licence, a change of name or address in the licence. By signing, the applicant also undertakes to obey the laws on road use and to pay the tolls, fees and fines arising from the vehicle.

Page 2 is the DLT power of attorney form, used when a representative files the application.

Who fills it in

  • the applicant — the vehicle owner or the licence holder;
  • if a representative goes to the DLT, the power of attorney on page 2 is also filled in and signed by the principal, the attorney and two witnesses.

What the form asks for

  • date (Buddhist Era year, พ.ศ.);
  • name, age, nationality, address in Thailand and phone of the applicant;
  • the request (มีความประสงค์) — up to four lines;
  • up to six attached documents;
  • the applicant’s handwritten signature with the name in capitals below;
  • on page 2 — details of the principal and the attorney and up to four lines of powers.

Documents usually attached

  • a copy of the passport (foreigner) or ID card (Thai citizen);
  • for a foreigner — a residence certificate from Immigration or an embassy;
  • the registration book or its copy — for vehicle requests;
  • a police report — if something was lost;
  • the old licence — when replacing a damaged one;
  • the power of attorney and a copy of the attorney’s ID — if a representative files.

The exact list, fees and processing time are not on the form — check with the DLT office where the vehicle is registered or the licence was issued.
