What this document is

“บันทึกถ้อยคำ กรณีแจ้งเอกสารหรือใบอนุญาตขับรถสูญหาย” is a written statement about the loss of a driving licence or another document of the Thai Department of Land Transport (กรมการขนส่งทางบก, DLT). It is addressed to the Registrar (นายทะเบียน) and recorded at the land transport office when you apply for a replacement licence (ใบแทน).

The form is in Thai only, has one page and no form code. It holds the holder’s details, which licence was lost, the circumstances of the loss and a declaration, signed by the holder and by the receiving officer.

About the “สขพ.5” header

The form prints “งานใบอนุญาตขับรถ สขพ.5” — the driving licence unit of Bangkok Land Transport Office Area 5 (สำนักงานขนส่งกรุงเทพมหานครพื้นที่ 5), located at the DLT headquarters on Phahonyothin Road, Chatuchak. It is that office’s version, but the DLT publishes it on its website for everyone.

Other offices commonly use the same statement or hand out their own copy at the counter — check with the office where you apply.

Who files it

  • a holder of a Thai private car or motorcycle licence who lost it, including a foreigner;
  • a holder who lost another DLT document (the form has an “other” option);
  • for a public vehicle licence (taxi etc.), Thai media report that a police report is also required — check with the office.

What the form asks for

  • date (the year is printed as “พ.ศ.25…”, so only the last two digits are written, e.g. 69 for 2569 = 2026);
  • title and full name, age, nationality, race (เชื้อชาติ), 13-digit identification number (Thai ID or a foreigner’s 13-digit number, if any);
  • address: house number, village or building, room/floor, moo, soi, road, khwaeng/tambon, khet/amphoe, province;
  • which licence was lost: private car or motorcycle — temporary, 1-year, 5-year or lifetime — or other (specify);
  • how, where and when the loss happened;
  • signature of the licence holder with the name in brackets; the receiving officer signs too;
  • at the bottom: licence number, type, issue date and expiry date, if known.

The form states that giving false information to an official is an offence under Section 137 of the Criminal Code — as printed on the form, punishable by up to 6 months’ imprisonment, a fine of up to 1,000 baht, or both.

Documents usually brought along

  • passport with visa (for foreigners), plus copies;
  • proof of address in Thailand if the office asks for it — for example a residence certificate or a work permit;
  • for a public vehicle licence, a police report;
  • a copy of the lost licence or its number, if you still have it.

Thai media reported in 2021 that no police report is needed for a private licence — the statement is recorded at the office instead — and fees of about 205 baht (5 baht application, 100 baht replacement, 100 baht photo and printing). Check current fees and requirements with the office; a queue can usually be booked in the DLT Smart Queue app.
