What this form is

The Visa Application Form is a one-page form of the Ministry of Foreign Affairs of Thailand, used when applying for a visa at a Royal Thai Embassy or Consulate-General abroad. It is in English only and is completed in BLOCK Latin letters.

The top-right corner asks for the visa type — Diplomatic, Official, Courtesy, Non-Immigrant, Tourist or Transit — and the number of entries.

Paper form or e-Visa

Many Thai embassies and consulates accept applications through the official e-Visa portal thaievisa.go.th. The paper form is needed where a mission accepts paper applications, and a filled PDF is a handy draft: all the data for an online application is in one place.

The procedure is set by the specific embassy or consulate — check its website before applying. Citizens of many countries may enter Thailand visa-free for a limited stay; the current list is published by the Thai MFA.

What the form asks for

  • title (Mr./Mrs./Miss) and full name as in the passport, former name if any;
  • nationality (current and at birth), place and date of birth, marital status;
  • travel document type and number, place and date of issue, expiry date;
  • occupation and employer, current and permanent address, phone, e-mail;
  • minor children if accompanying;
  • arrival date, means of travel, flight, length of stay, previous visit, purpose of visit;
  • address in Thailand and guarantors — at home and in Thailand, if any;
  • date and signature; for tourist and transit visas, a separate declaration that you will not work in Thailand.

The “For official use” box is completed by the embassy — leave it blank.

What is usually attached

  • passport — embassies usually require at least 6 months of remaining validity;
  • 2 photographs 3.5 × 4.5 cm taken within the last 6 months (printed on the form);
  • flight and accommodation bookings, proof of funds — depending on the visa category;
  • for Non-Immigrant visas, documents proving the purpose: employer or school letters, marriage documents and the like.

The exact list and the fee are published by the embassy you apply to. As printed on the form, the visa fee is non-refundable, even if the visa is refused.
