  • flight number, occupation, permanent address, address in Thailand;
  • name and address of a person of reference in Thailand;
  • minor children travelling on the same passport: names, dates and places of birth;
  • signature and date; a 4×6 cm photo is attached in the box. The FOR OFFICIAL USE ONLY part is completed by the officer.

Documents usually brought along

  • a passport with sufficient validity;
  • one 4×6 cm photograph;
  • Thai baht in cash for the fee;
  • proof of a return or onward ticket, accommodation and sufficient funds — the officer may ask for them.
