What this document is

STM.11 (แบบ สตม.๑๑) is a short letter to the Commissioner of the Immigration Bureau. A foreigner who has applied for a visa, a change of visa type or an extension of stay agrees that immigration officials may verify the facts of the application and undertakes to answer their questions and provide additional information.

The form is bilingual: Thai text on top, English below, with the same data in both parts. 2025 edition.

When it is submitted

  • with an application for extension of stay (TM.7) — for retirement, marriage, work, study and other grounds;
  • with an application for change of visa type (TM.86) or for a visa in Thailand (TM.87);
  • the exact package depends on the office: check whether STM.11 is required for your ground.

What the form asks for

  • date (Buddhist Era year, พ.ศ., in the Thai part; Gregorian year in the English part);
  • title (Mr./Mrs./Miss), full name, age, nationality, passport number, phone;
  • type of application: visa, change of visa type or extension (and for how many years);
  • reason, position and company — if the application is work-related;
  • coordinator and phone — a person through whom immigration can verify details (e.g. HR or your spouse);
  • the applicant's signature — by hand.

What the consent means

By signing, you allow immigration to verify the information in your application — for example, to call you or the coordinator, or to visit your home or company. Refusing to cooperate may affect the decision on the application.

Is STM.11 needed for a retirement or marriage extension?

Who is the coordinator?

A person through whom immigration can verify details: the company's HR officer, your Thai spouse or an agent. If there is none, the box can stay empty.

Which year do I write in the date?

In the Thai part — the Buddhist Era year (Gregorian + 543); in the English part — the Gregorian year. FoxDocs fills in both.

What goes into "extension for … year(s)"?

Usually 1: extensions for retirement, marriage, work or study are granted for one year. For a visa or change of visa the box stays empty.

Is there a fee for STM.11?

The form itself states no fee. The fee is paid for the main application (e.g. the extension) — check it with the office.
