What this document is
The “Criteria and Conditions Acknowledgement Form for Pre-approval Process on the LTR / Single Window for Visa and Work Permit System” is a bilingual (Thai/English) form addressed to the Commissioner of the Thai Immigration Bureau. It is signed by a foreigner who is applying to the Board of Investment (BOI) for qualification endorsement for the Long-Term Resident (LTR) visa and goes through pre-approval on the LTR / Single Window system.
By signing, the applicant confirms that the BOI officer has explained the pre-approval criteria and conditions, and undertakes to upload the missing documents to the system before receiving the LTR visa and to present them to the immigration officer at the One Stop Service Center for Visa and Work Permit. It is not a visa application.
Documents to upload (as printed on the form)
- the current visa, already cancelled by the Immigration, with enough remaining permission to stay after the cancellation to receive the LTR visa at the One Stop Service Center;
- the original and a photocopy of TM.6 if you last entered Thailand before 1 July 2022 or through a land border checkpoint;
- the original TM.47 receipt from your most recent 90-day notification.
What the form asks for
- the date of signing — the year in the Buddhist Era (พ.ศ.);
- title (นาย/นาง/นางสาว — Mr./Mrs./Miss), full name as in the passport, age;
- nationality and passport (or document in lieu of passport) number;
- the applicant's signature — by hand.
