What this document is
TM.94 (ตม.94 คำขอรับการตรวจลงตรา) is a foreigner's application to the Thai Immigration Bureau for a visa in the category "special temporary stay — long-term resident" (Long-Term Resident, LTR). The form is an attachment to Immigration Bureau Order no. 150/2565 of 22 August 2022 and is published on the official LTR programme website of the Board of Investment (BOI). It is bilingual (Thai and English), two pages.
Who files it and where
- a foreigner in Thailand who has received a BOI endorsement under one of the LTR categories: Wealthy Global Citizen, Wealthy Pensioner, Work-from-Thailand Professional, Highly-Skilled Professional;
- family members (dependants) of an LTR holder, if included in the BOI endorsement; check with the office whether each needs a separate form;
- it is filed with Immigration in Thailand — usually at the One Stop Service Center for Visa and Work Permit in Bangkok or the office indicated by BOI; confirm the place for your case with BOI or the office;
- according to the form, the applicant must submit it in person; the form states a fee of 50,000 baht, not refundable if refused. Check the current amount with the office.
What the form asks for
- the immigration office and the date of the application (Buddhist Era year, พ.ศ.);
- title (Mr./Mrs./Miss — circled), family name, first name, middle name, age, date and place of birth, nationality;
- passport number, date and place of issue, expiry date;
- current visa type, mode of transport, where you arrived from, port and date of last entry, TM.6 arrival card number (if any);
- the reason — your LTR category and basis;
- address in Thailand, phone, e-mail and details of the person who wrote the application (usually the applicant);
- signature or right thumb print — by hand; a 4×6 cm photo is attached.
What people usually bring
- passport and copies of the photo page, current visa and last entry stamp;
- the BOI LTR endorsement letter;
- one 4×6 cm photo (the form has a space for it);
- for family members — proof of relationship or marriage, if the office asks for it.
