What this document is
The Document Request Acknowledgement Form for LTR Application is a one-page form published by the Board of Investment (BOI) on the Long-Term Resident (LTR) visa portal. By signing it, the applicant confirms they are aware that the authorities involved may ask for additional documents or information while the LTR application is being processed.
It is not the LTR application itself and does not replace any of the listed documents — it only records that the applicant has been informed.
What the applicant acknowledges (as printed on the form)
- 1. One Stop Service Center for Visa and Work Permit (OSS), while the LTR application is in process: health insurance covering the entire stay (hospitalisation cover of at least USD 50,000, at least 10 months remaining) and a letter from the country of nationality or residence confirming no criminal record.
- 2. Department of Employment, while the work permit application is in process: employment certificate WP.46 signed by the company director with the company seal (if any) and the company registration certificate, with extra documents if the signer is a foreign director (A) or an authorised person (B, power of attorney with 10 baht stamp duty); and a recent medical certificate (not older than 1 month).
- 3. Immigration Bureau, during pre-approval for LTR visa issuance: a copy of the passport (data page and all pages with Thai stamps, spreads merged into one PDF), a copy of TM.6 if applicable, and original TM.47 receipts (90-day notifications).
What the form asks for
- the applicant's name as in the passport;
- the LTR application number assigned by the BOI system;
- a tick in each box of the three items;
- the applicant's signature — by hand.
Where it is submitted
The form is used in the LTR visa application process run by the BOI and the One Stop Service Center for Visa and Work Permit. How and when to upload or hand it in may change — check the current instructions on the BOI LTR portal or with the OSS.
