What this document is

The Affidavit is a one-page English statement in which a Belgian citizen declares, “under penalty of perjury”, the Belgian passport details, date and place of birth, present address in Thailand and monthly income in euros with an approximate amount in baht.

Such a statement is commonly used as proof of income for an extension of stay in Thailand based on retirement. The signature is legalised by the Embassy of Belgium in Bangkok.

The blank is published on the third-party website pattayathailande.com in the Belgian Embassy format; it is not a Thai government form. Check with the embassy that this format is accepted before you submit it.

Who files it

  • a Belgian citizen holding a valid Belgian passport (the nationality is printed on the form);
  • usually a retiree extending their stay in Thailand on the basis of income.

Citizens of other countries cannot use this form; their own embassy has its own income statement.

What the form asks for

  • item 1 — full name, Belgian passport number, date and place of issue, expiry date;
  • item 2 — date and place of birth;
  • item 3 — present address in Thailand;
  • item 4 — monthly income in EUR and approximately in THB;
  • item 5 — the name again: the declarant takes full responsibility for the veracity of the claims;
  • date and place of signing, signature (signed by hand).

Where to sign it and what to attach

The affidavit is usually signed and legalised at the Embassy of Belgium in Bangkok. The embassy generally asks for proof of the pension amount (for example statements from the pension authority or bank) — check the exact list, appointment procedure and fee with the embassy.

The legalised affidavit is then submitted to the Thai immigration office with the other documents for the extension. Immigration sets the income requirement and document list — check the current conditions with your office.
