What this document is
TM.18 (ตม.18, “คำร้องขอรับใบสำคัญถิ่นที่อยู่”) is the application of a foreigner who has already been permitted to reside permanently in Thailand to receive the Certificate of Residence (ใบสำคัญถิ่นที่อยู่, often called the “blue book”).
The English line on the form reads “Application for Substitute of Certificate of Residence”, while the Thai title and the declaration speak of obtaining the certificate. If you need a replacement for a lost certificate, ask the Immigration Bureau whether this form applies.
Who files it and where
- a foreigner who has been granted permanent residence (item 8 of the form);
- it is filed with the Thai Immigration Bureau, at the unit handling permanent residence; check the address and opening hours on the Bureau’s website;
- the form states that the application must be made in person.
What the form asks for
- family name, first name, middle name, age, other name (if any);
- nationality, race (เชื้อชาติ), occupation;
- place and date of birth, former domicile before entering Thailand;
- residence in Thailand: house number, road, tambon, amphoe, province;
- type of entry visa, date of entry and conveyance;
- passport number, date and place of issue; visa: embassy or consulate, number and date;
- children under 12 who entered with the applicant and children permitted residence together with the applicant;
- all dates in Thai format: day, month and Buddhist Era year (พ.ศ.).
What to attach
- two 4 × 6 cm photographs — required by the text of the form;
- passport and copies;
- documents on the permanent residence decision and other papers — as requested by the Bureau.
The exact document list, fees and processing time depend on the office — check with the office before you go. The signature or right thumb print is added by hand; the official boxes (height, scar, certificate number, order) are completed by the Bureau.
