What this document is
The Lost or Stolen Passport Form (แบบแจ้งพาสปอร์ตสูญหายหรือถูกขโมย, annex “ผนวก ก.”) is an official form of the Thai Immigration Bureau. It is filled in by a foreigner whose passport was lost or stolen in Thailand.
Using the details on the form, the immigration officer finds the last entry and the permission of stay granted on the old passport in the immigration system and records them — that is what the “FOR OFFICIAL ONLY” part at the bottom is for. The form is usually submitted together with a request to transfer the entry and visa stamps to a new passport.
What the form asks for
- title (Mr./Mrs./Miss) and name as in the passport;
- nationality;
- number of the lost or stolen passport;
- date of last entry to Thailand (date/month/year, Gregorian calendar);
- how you entered: by air (airport and flight number), by bus, train or boat (border checkpoint);
- last port before Thailand;
- type of visa: Non-Immigrant, Tourist (60 days), Transit, TR-15, 30 or 90 days;
- signature and date — the signature is made by hand.
What is usually attached
- a police report on the loss or theft of the passport;
- the new passport or travel document issued by your embassy, and copies;
- if available, a copy of the lost passport: data page, visa and entry stamp.
