What this document is

STM.5 (แบบ สตม.๕, Rules and Conditions Acknowledgment Form for the Temporary Residence Permit in the Kingdom, Non-O-X) is addressed to the Commissioner of the Immigration Bureau. The applicant for the long-stay Non-O-X visa confirms that the officer explained the conditions under which the permit of stay is terminated.

The form is bilingual (Thai and English), two pages, and is signed in front of an immigration officer together with the TM.89 application.

Which conditions you acknowledge

According to the form, the permit is terminated if:

  1. in the first year the savings account holds less than 3,000,000 baht;
  2. from the second year the account holds less than 1,500,000 baht and the money was not spent on medical care, a condominium, a vehicle or children’s education in Thailand;
  3. there is no health insurance;
  4. the applicant works without permission;
  5. the applicant poses a threat to society, peace or the security of the Kingdom;
  6. for the spouse and children: their permit ends together with the main applicant’s; for children also if they have no insurance or are over 20.

Every year the applicant must come to the immigration office and have the documents inspected.

Who files it and where

  • a foreigner applying for the Non-O-X visa with form TM.89;
  • it is signed at the immigration office in front of the officer, usually on the day TM.89 is filed;
  • there is no separate fee for STM.5 — the fee is paid for the TM.89 application.

What the form asks for

  • the immigration office and the date (Buddhist Era year, พ.ศ.);
  • title (Mr./Mrs./Ms.), full name, age, nationality;
  • passport number and its date of issue;
  • the applicant’s signature — by hand, in front of the officer.
