What this document is

STM.4 (แบบ สตม.4, “Certification Form for Education and Work Experience”) is an official form of the Thai Immigration Bureau. In it, the Thai employer certifies that the foreigner it wants to hire has the education and work experience required for the position stated in the work permit application.

The form is meant for a foreigner who applies to change the type of visa to Non-Immigrant B (business/employment) while in Thailand. It is signed by the employer, the applicant and two witnesses. The form warns that false information is prosecuted under Thai law.

Who signs it

  • the employer — according to the note on the form, an authorised person who signs under the company registration certificate;
  • the applicant — the foreigner being hired;
  • two witnesses.

What the form asks for

  • the applicant's title (Mr./Mrs./Miss), name, passport number, age and nationality;
  • the name and position of the employer's signatory and the name of the company, partnership or shop;
  • the position the foreigner is hired for;
  • education: highest degree, field of study, institution, country and year;
  • work experience: up to three previous jobs — position, employer and years;
  • signatures — by hand.

What to prepare

  • documents confirming previous work experience, if available;
  • the company registration certificate showing who may sign for the company.
