What this document is

This is the work permit application (คำขออนุญาตทำงาน) of the Department of Employment, Ministry of Labour of Thailand (DOE). DOE publishes it among its MOU forms — for foreign workers hired under agreements or memoranda of understanding (MOU) on labour cooperation between Thailand and other countries.

The form is one page, in Thai with English captions. The bottom part and the box in the header are for official use only.

Who files it and where

  • the applicant is the foreign worker, who signs the application;
  • at the end of the form the employer certifies that the applicant is really their employee, and signs;
