What this document is

Form BT. 55 (แบบ บต. ๕๕, "แบบแจ้งเข้าทำงานของคนต่างด้าว ตามมาตรา ๖๔/๒") is a form of the Thai Department of Employment. According to its heading, it is for foreigners who have been permitted to work: the foreigner informs the registrar which employer they have started working for, from what date and where.

The form has two pages: the front page is the notification itself, the back page lists the supporting documents and is completed by the office.

Who fills it in and where it is filed

  • the foreign worker who has been permitted to work — personally or through an authorised person;
  • it is received by the registrar (นายทะเบียน) of the Department of Employment; check with your provincial or Bangkok area Employment Office where and within what period it must be filed;
  • the form does not state a deadline or fee — check them with the office.

What the form asks for

  • date of the notification (Buddhist Era year, พ.ศ.);
  • worker's title, name, nationality, sex, age and passport (or document in lieu) number;
  • current address in Thailand and phone;
  • employer's name and Thai ID number (individual) or juristic person registration number (company), type of business;
  • date work started, work permit number and main nature of work;
  • workplace address and phone;
  • the worker's handwritten signature.

Documents to attach (listed on the back of the form)

  • filing in person: a copy of the passport, temporary passport, document in lieu of passport or other document issued by the Thai authorities;
  • filing through an authorised person: a power of attorney with the full stamp duty affixed;
  • plus a copy of the principal's passport or document in lieu, and the ID card of the authorised person (the officer makes the copy), or other documents if required.
