What this document is

The Smart Visa Termination Request Form is a one-page English form of the Board of Investment of Thailand (BOI) used to ask for the termination of a Smart Visa. Once the request is processed, BOI issues a Smart Visa Termination Letter.

The form itself warns that after the termination letter is issued, none of its details can be changed.

Who files it

  • a Smart Visa holder (Smart “T”, “I”, “E”, “S” or “O”) who resigns or whose employment with the current company ends;
  • an accompanying family member whose visa ends because the main Smart Visa holder’s visa is terminated;
  • a holder who changes to another visa type (for example Non-Immigrant B or ED), no longer meets the Smart Visa qualifications, or has another reason.

The form is signed by the applicant and by an authorised person of the company. Check the current procedure with the BOI Smart Visa Unit.

What the form asks for

  • Part A: full English name as in the passport, passport number, date of issue and expiry (dd-mm-yyyy), home address, phone, e-mail, current Smart Visa category and its expiry date;
  • Part B: the appointment date for termination (it cannot be rescheduled) and one or more reasons; when changing visa type — the new visa, the reason and where the new visa will be issued;
  • a checklist of attached documents, the signatures of the applicant and of the company’s authorised person, names and dates.

What to attach

  • a termination letter issued by the employer, signed by an authorised person of the company (as listed in the certificate of incorporation) and stamped with the company seal;
  • a copy of the Smart Visa stamp;
  • a copy of the latest entry stamp into Thailand;
  • when changing to a Non-B visa — the certificate of incorporation and the employment contract; to a Non-ED visa — a letter from the school stating the duration of study.

The form does not state a fee or processing time — check them with BOI.
