What this document is

The Application for Travel Record (คำขอคัดสำเนาข้อมูลการเดินทางเข้า-ออกราชอาณาจักร) is the Thai Immigration Bureau form used to request a copy of a person’s entries into and exits from Thailand for a chosen period.

The form is in Thai with English subtitles and is addressed to the Commander of the Immigration Technology Center. The official PDF has two versions: a request for your own record and a request for another person’s record under a power of attorney. This page fills in the version for your own record. The framed lower part is for official use only.

Who files it

  • a foreigner (or other traveller) who needs an official history of their own entries into and exits from Thailand;
  • typical reasons are visa or permanent residence applications, requests from an embassy, an employer or a tax authority — the form simply asks you to state the purpose;
  • to request another person’s record, the official form uses a separate version with a power of attorney and copies of ID documents of both people; it is not covered by this page.

What the form asks for

  • place and date of writing (Buddhist Era year, พ.ศ.);
  • title, first name and family name, nationality, age and date of birth;
  • Thai ID card number if you have one, passport number;
  • present address in Thailand (house number, road, tambon, amphoe, province, post code) and phone;
  • the period of the record (since — until) and its purpose;
  • the attachments: copy of passport and, if needed, other documents;
  • signature or right thumb print by hand.

  • a copy of your passport, as listed on the form;
  • other documents if relevant, for example a copy of a previous passport when the period covers it.

Where exactly to submit the request, the full document list, any fee and the processing time are not stated on the form — check with the Immigration Bureau or your immigration office in advance.
