What this document is
The Letter of Consent (หนังสือให้ความยินยอม) is a form of the Thai Ministry of Foreign Affairs. The father or mother gives written consent for the child to apply for a Thai passport and travel abroad, and agrees that an undertaking is made with the Ministry that the traveller is responsible for expenses incurred during the trip.
The form is bilingual (Thai and English), one page.
Who files it
- a parent (father or mother) of a Thai national child who is applying for a Thai passport;
- according to the remark on the form, the person exercising parental power (legal guardian) may also give consent; the signature lines on the form are only for father and mother, so check this case with the passport office or embassy.
The form does not list when exactly consent is required (for example, when one parent does not attend) — check with the passport office or embassy.
What the form asks for
- place and date of writing (Buddhist Era year, พ.ศ.);
- the parent’s title and full name;
- the parent’s address: house no., moo, street, sub-district, district, province/city, country;
- whether you are the father or the mother (the other word is struck out);
- the child’s title and name (under 15: ด.ช./ด.ญ.).
The parent signs by hand in the presence of an official. The certification block (“I hereby certify…”), the official’s signature, position and seal are completed by the office.
Where to certify it and what to attach
- sign the consent in person before an official at a district office (สำนักงานเขต/อำเภอ) in Thailand or at a Royal Thai Embassy / Consulate-General abroad — they certify it and affix the seal;
- attach a copy of the parent’s Thai ID card or passport, certified as a true copy (as the form requires).
The form does not list the other documents for the child’s passport, fees or processing times — check with the Department of Consular Affairs passport office or the embassy.
If a child under 15 is brought to apply by a third adult without the parents, remark 2 requires a power of attorney, also made at a district office or embassy. That is a separate document.
