What this document is

This is the power of attorney form (หนังสือมอบอำนาจ) published by the Department of Employment, Ministry of Labour of Thailand (กรมการจัดหางาน). With it the grantor authorises another person to act on their behalf in work permit matters: to sign the work permit application documents and to correct details in them.

The form is one page, printed in Thai and English. The scope of authority is already printed; the remark at the bottom says that a different power of attorney form should be used to limit the authority.

Who fills it in and where it is filed

  • the grantor — the foreign worker or the employer (a company signs through an authorised director);
  • the grantee — usually an employee of the employer (HR) or of a visa agency; the form asks for the grantee's position and office address;

  • copies of the grantor's and grantee's identity documents (passport or Thai ID card), signed as true copies;
  • if the grantor is a company — its registration documents and proof of the signatory's authority;
  • the work permit documents the grantee is filing.
