What this document is
A Thai birth certificate (สูติบัตร, form ท.ร.1 ตอน 1 / Thor. Ror. 1, Part 1) is issued in Thai by the district or local registration office. When it is needed abroad or by an authority that does not read Thai, an English translation is attached to it.
This page fills in the English translation template “Translation — Certificate of Birth”, which follows the layout of the Thai form box by box: the newborn, the mother, the father, the person who notified the birth and the registrar. The template is published on the DITP One Stop Service website.
Who uses it
- parents of a child born in Thailand who need the birth certificate in English (for example, for a passport, citizenship or registration in another country);
- adults born in Thailand who need their own birth certificate translated (visa, marriage, study or work abroad).
The translation does not replace the original certificate: it is submitted together with the original or a copy of it.
What the template asks for
- registration office and the newborn’s personal ID number;
- name, surname, sex, nationality, date and time of birth, the Thai lunar date (if shown), place of birth;
- birth order, who performed the delivery, weight at birth, the house registration and house code where the child’s name was added;
- mother and father: name, surname, ID number, age at the time of birth, nationality, province and country of birth, residence;
- the person who notified the birth, their relationship to the child, and the registrar’s name and date.
What to prepare
- the Thai original birth certificate (or a clear copy) to copy the data from;
- passports of the child and the parents, to spell names in Latin letters exactly as in the passports.
Who must sign or certify the translation (the translator, a translation agency, the Department of Consular Affairs, an embassy) depends on the authority that will receive it. Fees and processing times are not stated on the template — check with that authority.
How to fill it in with FoxDocs

- Check every field against the original and download the PDF.
- Print it, have the translation signed under “Certified correct translation” and submit it together with the original or a copy.
