What this document is
“Hoja padronal — Padrón Municipal de Habitantes” is the official form of Madrid City Council (Ayuntamiento de Madrid) to register in the municipal register of inhabitants (empadronamiento) or to change address within the city. The form is in Spanish and has two A4 landscape pages: the first is filled in, the second contains instructions, education level codes, the legal basis and the list of documents.
One sheet covers up to three people living at the same address. FoxDocs only types your values into the fields of the official PDF and does not change its layout.
Who files it and where
- anyone living in Madrid, foreigners included — on first registration, moving from another municipality or country, changing address within Madrid, a birth, or if previously not registered (omisión);
- it is filed at the Oficinas de Atención a la Ciudadanía Línea Madrid with an appointment (cita previa): phone 010 or 914 800 010, or madrid.es/citaprevia;
- the city’s official pages list no fee for registration; the registration certificate (certificado de empadronamiento) is issued free of charge;
- non-EU foreigners without permanent residence must renew their registration every two years;
- every adult listed on the sheet signs it by hand.
What the form asks for
- the dwelling address: street type and name, number, portal, staircase, floor, door, landline;
- for each of up to three people: name, first and second surname, sex, date of birth (dd/mm/aaaa), country, municipality and province of birth, nationality;
- ID document: type (DNI, passport, NIE, residence card — T/P/C), number and letter; for EU citizens the Pas/Doc box; education level code;
- cause of registration: omission, change of address within Madrid, move from another municipality or country, birth; previous province or consulate and municipality or country;
- mobile, e-mail and signature of each adult; number of people on the sheet;
- the blocks “Autorización de empadronamiento” (authorisation by a registered resident or the owner), “Autorización progenitores/representantes legales” (for minors registered at an address other than a parent’s), “Declaración de residencia” (when nobody has a document proving use of the dwelling), documents presented and the line “Madrid, a … de … de 20…”.
Documents usually brought along
- the completed hoja padronal signed by all adults;
- a valid ID for each person: DNI, passport, NIE or residence card;
- proof of use of the dwelling: a rental contract of at least 6 months or the title deed, and in some cases a utility bill as well;
- for minors: libro de familia or birth certificate;
- if the dwelling belongs to someone else: their authorisation (the “Autorización de empadronamiento” block) and a copy of their ID;
- check the exact list on madrid.es before your appointment.
How to fill it in with FoxDocs
